How to schedule a Home visit:
When you contact Dr. John, please let her know what services you require, and she will determine if she can be of assistance to you.
Once you have been given an appointment date and time, please Download, Read and Sign the Medicare Private Contract Form. Please have your signed form ready at the time of your appointment, this will allow for more time to focus on assisting you.
Please use the secure online payment options to make a deposit which is half of the total consultation fee, the remainder half can be paid at the end of the visit. I do not charge a travel fee.
Before Your Visit:
Please make your initial payment deposit. This is required as I do not charge a travel fee, and will be reserving a significant amount of time towards your appointment.
Medicare recipients, please fill the Medicare Contract Form. Medicare requires that you and your provider, if they are not participating with Medicare, sign this form. The form is an assurance that Medicare will not be billed for the services rendered to you. Please also fill out this Patient Consent to Treatment Form online.
Have any pertinent medical documents relating to your visit available. If you were recently discharged from the hospital or rehabilitation facility, please have your Discharge Summary available. If you do not have a Discharge Summary available, please fill the Authorization for Release of Records form available under the ‘Forms’ section, so I may attempt to get your records.
On the day of your visit:
Please arrange for your consultation to occur in your living/sitting room. If you have a home health aide or assistant, their presence is welcome and needed, to assist you with transfers during your consultation.
Please remove any furry or exotic pets from the room where you will be seen by the doctor. Please do not smoke, or spray any air fresheners in the room where you will be examined. Thank you for your consideration.
It is best that there is not a crowd of people present during your appointment [outside of those people who will be directly assisting you during the consultation], to ensure HIPPA policies are maintained, and your doctor can work in a conducive environment, without hindrance.
At the end of your consultation, please pay the remainder of your consultation fee.
